Please find attached checklist with list of documents that you need to send in electronic version. Please note that we in case of passport we need scans of ALL PAGES.

In order to apply for WORK PERMIT for you I will also like to ask you to fill in 2 documents:

Power of Attorney (thanks to that we will be able to apply for your work permit in your name)
Form with data needed for application form
