Treatment options for unresectable disease are limited. Randomized trials have not shown a survival advantage after chemoembolization. The liver cannot tolerate high doses of radiation. The disease is not responsive to chemotherapy, including newer agents such as gemcytabine. Investigative immunotherapy and gene therapy techniques have not yet been successful. Based on the presence of hormone receptors on the tumor, tamoxifen has been tested, but without success, and octreotide has had some modest activity. In patients with resectable tumors, polyprenoic acid (a retinoic acid formulation) and intraarterial 131I-labeled lipiodol have been reported to reduce the rate of recurrence.
